In accordance with University Policy 1600.01 Requesting an Exception to the Normal Competitive Search Process (Requesting an Appointment of Opportunity), and Procedure 1600.01A Requesting a Dual Career Assistance (DCA) Appointment of Opportunity, this E-sign form is to be completed by a current Western employee or prospective employee (defined as a "Primary Employee" in Procedure 1600.01A), in order to request consideration for development of a new position for the employment of the primary employee's spouse or partner.
 
Prior to completing this E-sign form, as outlined in Procedure 1600.01A, the Primary Employee must first complete and submit a Requesting Dual Career Assistance e-sign form in order to request DCA consideration for the Primary Employee's spouse/partner, receiving all necessary approvals as indicated on the E-sign form.

Required Information
Primary Employee (current Western employee or prospective employee who is requesting DCA consideration for their spouse/partner)
 
The following information is only required for current employees. If a prospective employee, please enter W# as "N/A", provide position title on your offer letter, and department, and enter last three items as "N/A".
 
DCA Candidate (spouse/partner of Primary Employee)
 
Request for New Position
A detailed, current resume/CV for the DCA candidate is required and should be provided by the Primary Employee as an attachment to this e-sign form.
 
In as much detail as possible, the Primary Employee is to provide an outline of the new position that
they are requesting for potential development, in order to utilize the qualifications and occupational interests
of the DCA Candidate. This information may be attached as a separate document to this e-sign form.
 
If possible, details of the position being requested should include which department or organization unit
the new position may be located in and provide specific duties that the position would have responsibility for.
 
The Primary Employee should also articulate how the DCA Candidate is highly qualified to fulfill the proposed duties of the position.
 
Required Approvals
NOTE TO APPROVERS:
In order to complete the process, all parties receiving this form should advance the form using the "Approve" button, however, the approver comments should clearly indicate the nature of the decision (e.g., recommendation to proceed or not with creating new position).
 
Requesting Department Chair or Director (Primary Employee's Department Chair/Director)
 
Within 5 working days of receiving this form from the Office of Civil Rights and Title IX Compliance, the Requesting Department Chair/Director must review the request from the Primary Employee and advance the form by hitting the "approve" button on the e-sign form to the Requesting Dean or VP, including written rationale on the e-sign form indicating their recommendation to proceed or not. The Requesting Department Chair/Director should coordinate with the Primary Employee to gather additional information, if needed, in order to inform their recommendation.
 
 
Requesting Dean or Vice President (Primary Employee's Dean or VP)
 
Within 5 working days of receiving this form, the Requesting Dean/Vice President must review the Department Chair/Director's recommendation, and advance the form by hitting the "approve" button on the e-sign form to the Receiving Dean or VP, including written rationale on the e-sign form indicating their recommendation to proceed or not. The Requesting Dean/VP should coordinate with the Requesting Department Chair/Director and the Primary Employee to gather additional information, if needed, in order to inform their recommendation.
 
 
Receiving Dean or Vice President (Dean or VP whose College/VP unit the proposed new position falls in)
 
Within 5 working days of receiving this form, the Receiving Dean/Vice President must review the Requesting Dean/VP's recommendation, and advance the form by hitting the "approve" button on the e-sign form to the Receiving Department Chair/Director, including written rationale on the e-sign form indicating their recommendation to proceed or not to proceed with the creation of a new position. The Receiving Dean/VP should coordinate with the Requesting Dean/VP, Requesting Department Chair/Director, and/or Primary Employee to request additional information, if needed, in order to inform their recommendation.
 
 
Receiving Department Chair or Director (Department Chair or Director whose Department the proposed new position falls in)
 
Within 15 working days of receiving this form, the Receiving Department Chair/Director must review the Receiving Dean/VP's recommendation, and advance the form by hitting the "approve" button on the e-sign form to the Office of Civil Rights and Title IX Compliance, including written rationale on the e-sign form indicating their recommendation to proceed or not proceed with the creation of the new position. If recommending to proceed, the Receiving Department Chair must attach to the e-sign form a draft position description that has been developed in collaboration with the Receiving Dean/VP, CRTC, and with HR (for staff positions). The Receiving Department Chair/Director should also coordinate with the Requesting Dean/VP, Requesting Department Chair/Director, and/or Primary Employee to request additional information, if needed, in order to inform their recommendation.
 
 
 
 
CRTC will review the recommendation from the Receiving unit, including the draft position description if applicable.
 
Whether creation of the new position is approved or disapproved, CRTC will route the e-sign form to Human Resources. In doing so, CRTC will prompt the following notifications to occur before the form routes to Human Resources, as described in Procedure 1600.01A Requesting a Dual Career Assistance Appointment of Opportunity:
  • Receiving Dean/VP - Notifies the Requesting Dean/VP and Requesting and Receiving Department Chairs/Directors whether or not the new position has been approved for creation, and if disapproved, the rationale for denying the request. If the position is not approved, any funding identified for the new position throughout the previous approval process will be returned to the original source.
  • Requesting Dean or requesting VP or requesting VP designee - Notifies the Primary Employee in writing if creation of the position is approved or disapproved. If disapproved, the notification will include the rationale provided by the Receiving unit for denying the request.
 
If creation of the new position is approved, Human Resources will post the new position to the University's DCA job posting listserv for DCA candidates to review/consider applying. HR will then lock the e-sign form. For classified and pro-staff positions, all collective bargaining agreement or handbook and associated policy requirements must be met before DCA Candidate applications may be considered.
 
If creation of the new position is not approved, Human Resources will lock the e-sign form.
Routing Instructions
Note: By pressing SUBMIT, this form will be submitted directly to Western Washington University's Office of Civil Rights and Title IX Compliance (CRTC@wwu.edu).
 
The CRTC Office will review the form to ensure all required information has been provided, initiate the routing sequence, and send a link to the form to the Initiator/Primary Employee so that they will have access to the form as it moves through the process.
