New Employee

• Employee Name: Employee name must match name on card.
• Preferred First Name: Complete this field to use your preferred first name in outlook and the Campus Directory..
• ID#: Indicate WWU Employee Identification if known (not required).
• SS#: Must match card.
• Date of Birth
• US Citizen
• Prior State Service: List name of agency/institution and dates of employment From/To (MM/DD/YR).

Home Address     List both mailing address and physical address if different.

Office Address     Campus address required – Contact supervisor if information not known.

Degree Information         Complete highest degree obtained, date received, and discipline.

Department of Retirement Systems Verification – Please complete. RCW 41.50.130 requires the employer to obtain this information.
