In accordance with University Policy 1600.01 Requesting an Exception to the Normal Competitive Search Process (Requesting an Appointment of Opportunity), and Procedure 1600.01A Requesting a Dual Career Assistance (DCA) Appointment of Opportunity, this E-sign form is to be completed by a current Western employee or prospective employee (defined as a "Primary Employee" in Procedure 1600.01A), in order to request DCA consideration for the Primary Employee's spouse/partner.

Dual Career Assistance may be requested by faculty, professional or classified staff, and executive officers, and is intended as a recruitment or retention measure for the Primary Employee.

In accordance with PRO-1600.01A, requests for DCA require approval from the Primary Employee's Department Chair/Director and Dean/Vice President.
 
Upon completion of this form receiving all of the necessary approvals, as indicated below in the routing instructions, the Primary Employee and/or their spouse/partner's contact information will be added by Human Resources to an e-mail listserv so they can be informed of new or vacant employment postings available at Western, to which the spouse/partner may apply as a DCA candidate. Applicable jobs will be held for 5 business days before being advertised publicly (see Policy 1600.01 for exceptions). DCA requests for new or vacant positions are only available during the 5-day DCA posting and are no longer available once a position has been advertised externally.
 
The Primary Employee may periodically be asked to submit an updated copy of this "Requesting Dual Career Assistance" E-sign form for approvals in order for their spouse/partner to continue to be subscribed to the e-mail listsev in order to be notified of DCA opportunities.

Following completion of this initial E-sign form, the Primary Employee, if interested, may also complete and submit a Requesting Dual Career Assistance - Development of New Position e-sign form, in order to request consideration for development of a new position for the employment of their spouse/partner as a DCA opportunity.

The following information is only required for current employees. If a prospective employee, please enter W# as "N/A", provide position title on your offer letter, and department, and enter last three items as "N/A".

Contact Information
Please provide the email address(es) you would like Human Resources to use in adding your spouse/partner to the listserv to be informed of new internal employment postings available at Western, for which they may apply as a DCA candidate. A phone number is also requested in case there are any questions processing your request. Additional contact information is optional.

Department Chair or Director
 
Within 5 working days of receiving this form from the CRTC, the Department Chair/Director must review the information provided on the form by the Primary Employee and advance the e-form, either approving or disapproving the request. The decision to approve or disapprove the request should be based on their desire to recruit/retain the Primary Employee.
 
If the Department Chair/Director approves the e-sign form, they route it to the Primary Employee's Dean/Vice President, indicating on the e-sign form their recommendation to recruit/retain the Primary Employee.
 
In the case of a Department Chair/Director disapproving a request for DCA consideration, they must first consult with the Office of Civil Rights and Title IX Compliance (360-650-3307 or CRTC@wwu.edu ). Upon disapproving the request for DCA consideration, they will need to provide written rationale for disapproval on the e-sign form, and route it to the Office of Civil Rights and Title IX Compliance (CRTC@wwu.edu).
 
 
Dean or Vice President
 
Within 5 working days of receiving this form, the Dean/Vice President must review the Department Chair/Director's recommendation to endorse the Primary Employee's request for DCA, and make the decision on whether to approve or disapprove the request, based on their desire to recruit/retain the Primary Employee.
 
If the Dean/Vice President approves the e-sign form, they route it to the Office of Civil Rights and Title IX Compliance (CRTC@wwu.edu).
 
In the case of a Dean/Vice President disapproving a request for DCA consideration, they must first consult with the Office of Civil Rights and Title IX Compliance (360-650-3307 or CRTC@wwu.edu ). Upon disapproving the request for DCA consideration, they will need to provide written rationale for disapproval on the e-sign form, and route it to the Office of Civil Rights and Title IX Compliance (CRTC@wwu.edu).

Note: The Initiator/Primary Employee is not required to enter an email address below to submit. By pressing SUBMIT, this form will be submitted directly to Western Washington University's Office of Civil Rights and Title IX Compliance (CRTC@wwu.edu).
 
Upon initial receipt of the form, the CRTC Office will review the form to ensure all required information has been provided, initiate the routing sequence, and send a link to the form to the Initiator/Primary Employee so that they will have access to the form as it moves through the process.
